Patient Portal

We offer our patients secure internet access to their own medical information on line, so they may view their personal health record at any time or place with internet access.

GET WEB-ENABLED:
You will have access to our secure server with your user name and password. You will be given a user name and password at the time of your visit, and will receive periodic updates through your personal e-mail address on file.

  • Request appointments (for non-urgent issues)

  • Send messages to clinical staff

IMPORTANT!
Please provide us with a non-work/non-employment related e-mail address, and you may then access your personal health record from a Smart Phone or any computer with an internet connection.

What to Bring to the Appointment

  • Insurance card
  • Photo ID
  • Physician referral form if required by your insurance provider
  • A list of medications including the dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent EEG's, EMG's, CT scans, MRI's or other test results related to your condition
  • Means of payment for any co-pay, co-insurance or unmet deductible
  • Completed patient forms

Insurance and Payment Information

Shore Neurology, PA is a participating provider for Medicare and most major insurance plans. We are happy to process all insurance forms for you as long as we are provided with complete and accurate information about your insurance coverage. We encourage all patients to confirm their insurance coverage prior to making an appointment with our practice. We expect payment promptly if you receive a bill from our office for balances that may be due. Please feel free to discuss any questions you may have about billing with office staff.

Depending on your insurance coverage, you may be responsible for outstanding deductibles, co-payments and/or co-insurance. Your insurance company may require you to provide a co-payment at the time of your appointment. You are responsible for any charges not covered by your insurance provider and we request payment for these services at the time of your appointment. When necessary, our staff will work with patients who may require a payment plan.

If you have questions regarding billing or which insurance plans we accept, please call us at (732) 240-4787, option 6. Questions regarding insurance coverage and benefits should be directed to your employer or insurance provider.

We are currently unable to accept patients for medical care who are involved with litigation or have been injured or ill as a result of a work-related accident or a motor vehicle accident.

Appointments

We see patients by appointment only. Although we work very hard at running on time, and even though we leave time in our schedules to accommodate delays, we will sometimes be unavoidably late and we apologize in advance.

Please remember that your appointment time is valuable. If you are unable to make your appointment, please let us know at least 24 hours in advance so we may schedule someone else that needs care. Without 24 hours notice, we may charge a fee of $25 for a missed appointment or $50+ for a missed testing procedure.

Prescription Refills

Requests for prescription refills are renewed during normal office hours and may be made through our Patient Portal. In some instances, we may require that the patient be seen in our office prior to the medication being refilled. Please allow at least 48 hours during our normal business hours for your request to be processed. No requests will be processed after 12 noon on Fridays.

Privacy Policy

The Shore Neurology, P.A. Privacy Policy describes how protected health information may or may not be used by our practice and describes patient rights regarding their health information.
